Wellness Plan Agreement
By enrolling in this wellness plan, you agree to the terms and conditions outlined below. These terms govern the relationship between you, the client, and Aurora Vitalis, ensuring a safe, respectful, and professional environment for all.
General Wellness Plan Terms

Wellness plan benefits, such as priority scheduling, extended appointment times, discounted services, and exclusive product discounts, are only available to active participants. Your plan must be current and in good standing to access these benefits.

Plan fees are billed monthly and are non-refundable.

You may cancel your wellness plan at any time; however, refunds will not be provided for the current month once payment has been made. Cancellation requests take effect the following billing cycle.

A 7-day grace period for monthly payments is offered. If payment is not made by the end of the grace period, your wellness plan will be canceled.

This wellness plan is not a substitute for traditional medical care or health insurance. By enrolling, you understand and agree that the care provided under this plan does not include diagnosis or treatment of medical conditions. I provide advice and recommendations but cannot legally diagnose or treat any health conditions.

Priority scheduling is subject to my office hours at Aurora Vitalis. Services are not offered outside of these hours. If you require urgent care or wellness advice outside normal business hours, you should seek treatment at an after-hours urgent care center or hospital emergency department.
Practitioner Rights
At Aurora Vitalis, I strive to provide exceptional care to all my clients. However, I reserve the right to refuse service or dismiss clients under the following extenuating circumstances, including but not limited to:
